Heart House Hospice Board, Staff and Volunteers work together to advocate for our
clients and their caregivers. We do this in a number of ways:
Heart House Care Coordinators provide information to clients, families and caregivers
about community resources, end of life care planning and companion clients, family
members and caregivers throughout the end of life journey.
Interact with community service agencies to insure hospice services can fit into
a continuum of care.
Participate on the Mississauga Halton Palliative Care Network and Central West Palliative
Care Network to advocate for and to work together with other palliative care providers
to develop a comprehensive seamless continuum of palliative care services.
Work with the Mississauga Halton Local Health Integration Network (MNLHIN) and the
Central West Local Health Integration Network to advocate for recognition of the
need for quality palliative care services in our communities.
Participate in provincial Hospice Association of Ontario initiatives and committees
to support the development of provincial standards of care in hospice services;
information sharing amongst hospice providers; education through the annual conference
and advocacy at the provincial government and community level.
Engage in community health fairs, education opportunities and public speaking to
inform the community about hospice services and need for end of life care planning.
Heart House Care Coordinators attend rounds at Credit Valley Hospital, The Trillium
Health Centre and William Osler Brampton Civic Site.
Heart House Care Coordinators are members of community resource teams in Brampton
and Mississauga that discuss client needs and problem solve pain and symptom management
Heart House participates on a number of other community committees for advocacy,
information sharing, education and development including: Metamorphosis, MHLHIN
Integration Advisory Group, Peel Volunteer Administrators Network, Infection Control
Network of Peel, Professional Administrators of Volunteers Resources Ontario, HRIS
Advisory Group and a number of task groups and subcommittees of the MHLHIN, CWLHIN,
Mississauga Halton Palliative Care Network and Central West Palliative Care Network.
Client and Community Education
Heart House provides information to the community, our clients and their families
about the options available for living with a life threatening illness to empower
and improve their quality of life.

Volunteers receive ongoing training provided ? times a year.
Volunteers attend agency rounds 3 times a year which provides an opportunity for
volunteers to share their experiences with each other, the care coordinators and
Community presentations upon request are made at schools, community health fairs,
agencies, clubs and other groups about end of life care planning and hospice services.
An agency Open House for service providers is held annually.
Brochures and promotional materials are made available to hospitals, doctorsï¿½ offices,
libraries and other public locations.
